Florida Book Challenges: Alachua County Public Schools

To Whom It May Concern:

Pursuant to the Florida Sunshine Law, I hereby request the following records:

1. A copy of any complaint on the Instructional/Library Material Re-Evaluation Form (Form 9130.01 F1) submitted on or after January 1, 2021.

2. A copy of any report, determination, or recommendation concerning any complaint referred to in Request No. 1, or any records otherwise generated pursuant to policy po9130.01. This request is likewise limited to records relating to complaints submitted on or after January 1, 2021.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
