I have attached a redacted copy of an Alabama death certificate. This is
a representation of the information we collect for deaths. Please notice
that the standard question regarding pregnancy if the deceased is female
is on the death certificate.

I also took the liberty of attaching two publications from the National
Center for Health Statistics (NCHS) which contains some information about
maternal mortality.

portion of this request should include reference to the data elements
collected by this department and should not include any personal

information.
