To Whom It May Concern:
Pursuant to the Alabama Public Records , I hereby request the following records:
Policies, procedures and/or documents pertaining to indexing for FamilySearch, otherwise known as Intellectual Reserve, Inc. or FamilySearch International, a genealogical non-profit operated by The Church of Jesus Christ of Latter-day Saints (LDS) by inmates within facilities run by the Alabama Department of Corrections . This includes any documents regarding Family History Centers operated within such facilities, and any documents referencing LDS Correctional Services, a division of LDS. Examples of such documents include contracts between the Alabama Department of Corrections and FamilySearch, often called a "Memorandum of Understanding," with an attachment describing FamilySearch's assistance.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days.
Sincerely,
Burkely Hermann

Good morning,
I am in receipt of your request for records. I have not received a completed Public Records Request form.
You will need to follow the Department's procedure to request public records by completing the Public Records Request Form, which I have attached for your convenience, (the request form can be found on our website at http://www.doc.alabama.gov/ under request public records for future reference) and submit it along with a $25.00 processing fee. The processing fee payment must be submitted in the form of a cashier's check or money order made payable to the Alabama Department of Corrections.
Once we receive your completed request form and processing fee, we will review our records for documents responsive to your request. If we determine there are records responsive to your request, we will notify you of the estimated costs to produce the records. Payment of the estimated costs must be paid before we begin production of the records. Upon completion, we will provide you with an invoice of any final costs. No records will be made available until the final costs are paid.
The procedure to request public records, including our fee schedule, is further explained in the public records request form.
Please note that we do not produce documents electronically in connection with public records requests.
Let me know if you have any questions.
Sincerely,

A fix is required to perfect the request.
From: Burkely Hermann
The check applies to the form sent by Mr. Salzberg on November 25, 2019. I am not sure why your Department did not realize this and made this error. Please fulfill this request immediately.
Best regards,
Burkely Hermann

Ms. Shingles,
Although I disagree with your assessment that I have not submitted the request since I requested documents originally on October 25, followed by Mr. Adrien Salzberg submitting the public records request form on my behalf on November 25, and my payment of processing fees on December 4th, as is documented on this chain of emails, I'll follow your procedures in hopes of getting some documents relevant to my request. I will shortly submit a request via email, not through this Muckrock system, as that would make personally identifiable information about myself public. Please be on the lookout for an email from bhermann@mail.com titled "Public Records Request - FamilySearch Indexing."
Best regards,
Burkely Hermann

Mr. Hermann,
Please mail your completed Public Records Request form and $25.00 processing fee together. Do not email the form. All of the request should be together.
