Pursuant to the Freedom of Information Act and Privacy Act, 5 U.S.C. ยง 552. I hereby request the following records:

All records with respect to the following criteria and within the timeframe of 2020 to 2023, the day this request is processed:

1. Investigative Records - Civil Rights violations, Constitutional violations, and HIPAA violations (if that part is known to the DOJ) with regards to AdventHealth aka Adventist Health System aka Adventist Health System Sunbelt Healthcare Corporation d/b/a AdventHealth (Healthcare Corporation) as the subject of investigation. Include AdventHealth leadership, if that is permissible, otherwise disregard this sentence.

2. Policy and Procedure Documents - Documents that explain the DOJ's Civil Rights Division's procedures, policies, and rules.

3. Case Documents - Documents that regard AdventHealth aka Adventist Health System aka Adventist Health System Sunbelt Healthcare Corporation d/b/a AdventHealth (Healthcare Corporation)

4. Reports and Statistical Data - This could include internal reports, audit results, statistics about civil rights complaints or cases, etc. in a general sense

5. Legal Opinions and Decisions - Of the DOJ, Civil Rights Decision that are the currently most revised or relied upon opinions and decisions of the DOJ that informs their decision-making with respect to civil rights.

6. Research Materials and Studies - List by title only, but do not produce these records/documents

If (1) through (6) produce results in excess of 500 results, please inform me of the number of results listed above by category number listed above, and I will revise the criteria so as not to delay the processing time.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I would also appreciate an approximate time frame for this request to be processed. Depending on the time frame for the request to be fulfilled and any fees that may be incurred, I may revise the criteria.

If you deny any part of this request, please cite each specific exemption you think justifies your refusal to release the information and notify me of appeal procedures available under the law.
