Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

The number of licensed and operating dialysis centers in Illinois, Cook County and Chicago.

A list of all dialysis center operators in Illinois.

Please provide a breakdown of the number of licensed and operating dialysis centers per zip code and who they are licensed to in the Chicago Metropolitan area.

For example, Fresenius, Davita, U.S. Renal Care, etc.

How many complaints have been filed with the state against dialysis centers operating statewide and in Chicago.

I'm a reporter at the Chicago Crusader newspaper and this request is made as part of news gathering and not for a commercial use.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter.

I look forward to receiving your response to this request within 5 business days, as the statute requires.

This email is in regards to your recent Freedom of Information Act request for records pertaining to licensed and operating dialysis centers in Illinois, Cook County, and Chicago.

The Department has extended the date in which your records must be provided to May 18, 2021. Section 3(e) of the Freedom of Information Act, 5 ILCS 140, permits the Department to extend the time for response if the request for records cannot be complied with by the public body within the time limits prescribed by the Act without unduly burdening or interfering with the operations of the public body.
